In the Alternative Bilateral Sacral Flexion MET, after the isometric contraction how should the barrier be engaged?

The patient relaxes and breathes.

Push farther into barrier for 3-5 seconds, repeat 3-5x.

Push away from the barrier for 5-8 seconds and then reassess.

In MET the contraction against a barrier is followed by a deliberate release that allows the body to reset toward a new barrier. For Alternative Bilateral Sacral Flexion, after the isometric contraction you should have the patient push away from the barrier for about 5–8 seconds. This direction reinforces a release and lengthening of the affected tissues, enabling the sacrum to move toward the new barrier. After this maneuver, you reassess the area to determine if a new, more neutral barrier can be established and whether another cycle is needed.

Relaxing and breathing is the relaxation phase, not how you engage the barrier. Pushing farther into the barrier would deepen the barrier rather than facilitate release. Oscillating at the barrier isn’t part of the MET sequence for this maneuver.

The patient oscillates at the barrier 10 times.
